If your status of Taiwan ARC is “Permanent（永久居留）”,please submit the following documents.

Sightseeing

１．Two Visa Application Form
２．Two photo; stick it on the application form（2in x 1.4in, taken in the last 6 months, face the camera directly without wearing a hat and must use a plain white background）
３．Passport（the original and 1 copy (photo page)）
４．Alien Resident Certificate（the original and 1 copy of both sides）
５．Bank Account Book（updated in the last 1 month ; the original and 1 copy of the pages which show the holder’s name and the last balance）or Certificate applied from the bank.

７．Round-trip flight ticket booking
８．Hotel Reservation
※If you participate in a package tour, please provide ①schedule of stay issued by the travel agent, ②flight ticket reservation PNR, ③travel contract with tour agency’s stamp.
※If the reservation was made by other person, please also provide his/her passport copy.
※If you stay at relative or friend’s home, please provide ①invitation letter from relative or friend (stating that he/she understand that you will be staying at his/her home), ②Letter of Guarantee from relative or friend, ③Certificate of Residence（住民票）（the original from relative or friend in Japan; issued within 3 months）and a copy of residence card from your relative or friend (foreigners only).
※ If someone will cover all the expenses for you, please provide Letter of Guarantee from them.

Company outing

１．The above 1 to 8（If the travel expenses are fully covered by the company, except for 5）
２．A document issued by your company（Please indicate purpose of visit, length of stay, description of the cost burden and put your company’s and the representative’s seals）

※Visa applicants may need to submit additional documents upon request.
※In accordance with regulations,　it is necessary to confirm that the applicant is residing in Taiwan at the time of application or receipt. If you wish to apply and receive the application by representative, the following documents are required in addition to the above.

3. Certificate of Entry and Exit Dates issued by National Immigration Agency, Ministry of the Interior
