Labor (domestic helper) (移工) ※家庭雇用

※It is necessary for the employer to travel together on the same flights with the visa applicant.

If your status of Taiwan ARC is “Labor（移工）”,please submit the following documents.

１．Visa Application Form
２．One photo; stick it on the application form（2in x 1.4in, taken in the last 6 months, face the camera directly without wearing a hat and must use a plain white background）
３．Passport（the original and 1 copy）
４．Alien Resident Certificate（the original and 1 copy of both sides）
５．Work permit from Ministry of Labor
６．Re-entry permit to Taiwan (the original and 1 copy）
７．Employer’s and care receiver’s Passport（the original and 1 copy）
８．Employer’s personal bank book or Certificate applied from the bank（updated in the last 1 month ; the original and 1 copy of the pages which show the holder’s name and the last balance）
９．A letter of guarantee from employer
10．Itinerary in Japan
11．Round-trip flight ticket booking of the visa applicant and the employer
※If you participate in a package tour, please provide ①schedule of stay issued by the travel agent, ②flight ticket reservation PNR, ③travel contract with travel agency’s stamp.
12. Hotel reservation

※Please print all the documents on A4-size papers. Please do not staple any document.
※Visa applicants may need to submit additional documents upon request.
※In accordance with regulations,　it is necessary to confirm that the applicant is residing in Taiwan at the time of application or receipt. If you wish to apply and receive the application by representative, the following documents are required in addition to the above.
1. Letter of Attorney (written by applicant)
2. Representative’s ID card (the original and 1 copy)
3. Certificate of Entry and Exit Dates issued by National Immigration Agency, Ministry of the Interior
